Documents signed by Gretchen Van Wye require a long form birth certificate with the exemplification letter.
New York Count Clerk Birth/Death Certificate Authentication: Your request for authentication has been denied due to the following:
- Insufficient fee/fee not included. (Money order only)
- Check is not signed, dated or made payable to “County Clerk, New York County”
- The Letter of Exemplification is missing and is required for authentication. Please obtain the letter of Exemplification from the Department of Health. (order online through Vitalchek.com)
- The certificate numbers or dates on the Letter of Exemplification do not match the certificate.
- The certificate is issued outside of the five boroughs of New York City. Please contact the county that issued the document for further instructions.
